Initial Comprehensive Assessment:
Every client receives a thorough assessment followed by a written report stating findings and recommendations. The assessment will include a discussion about the client’s health history, current status, present concerns, and desired health and wellness goals. Physical, emotional, and social care needs will be assessed. In many cases a home walk-through will be conducted together to look for safety hazards and potential fall risks.
Concierge health advocacy services:
Clients and caregivers will find peace of mind knowing they have an experienced Professional Independent Health Advocate available to them as a resource and someone who will be there as a healthcare partner and guide. Concierge advocacy services will coordinate an individual’s or family’s total health care needs in consultation with primary and secondary care providers.
This service is for people with ongoing needs, for people with busy schedules, working couples, and families who would like on-going healthcare support.
Coordination and supervision of in-home health services:
This service is a once monthly in-home review and assessment of existing home care, safety, medication management, and updates of provider care status and review. This service follows the initial assessment.
Aging in Place Consultation:
This service is for couples, Solo Seniors, new homeowners, or anyone who wishes to remain in their home and Age in Place. Aging-in-Place planning should not be delayed for our later years. It is a process that takes place over time.
With this service, clients will receive a thorough consultation by a Certified Aging-in-Place Specialist who will make recommendations specific to your needs and goals. This service takes place after the initial assessment.
Advocacy, Navigation, and Coaching:
After the initial assessment, this plan provides hourly advocacy support services to achieve the goals set forth in the comprehensive care plan of action.
Reassurance Calls and Home Visits:
Pre-arranged morning and/or evening reassurance calls provide confidence that you or your loved one is all right. Additionally, these check-ins with your Professional Independent Advocate are an opportunity to address potential concerns such as medications, nutrition, activity level, home safety, up-coming medical appointments and transportation needs.
This service is aimed at Solo Seniors, busy adult children of seniors, or long-distance family members.
Pre-hospitalization preparation and safety review:
Following an initial assessment, this service prepares a patient for their upcoming hospitalization. This step outlines expectations and levels of support.
